RESOLUTION AUTHORIZING (A) THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH SAMARITAN HOUSE TO CONTINUE TO PROVIDE CASE MANAGEMENT SERVICES, EXTENDING THE TERM BY ONE YEAR TO JUNE 30, 2010; INCREASING THE AMOUNT BY $40,000 TO $120,000; $40,000 IS NET COUNTY COST; AND (B) AUTHORIZING THE DIRECTOR OF THE HUMAN SERVICES AGENCY OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS
RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
WHEREAS, on July 10, 2007, the County Manager’s Office approved an Agreement with Samaritan House, Agreement Number 71000-08-C057, waiving the Request for Proposal (RFP) process, authorizing services in the amount of $40,000, for the term of July 1, 2007 to June 30, 2008; and
WHEREAS, on June 26, 2008, the County Manager’s Office approved an Amendment to the Agreement increasing the amount by $40,000 for a total Agreement amount of $80,000 and extending the term to June 30, 2009; and
WHEREAS, the Second Amendment adds $40,000 increasing the total obligation to $120,000 and extending the term by one year to June 30, 2010; and
WHEREAS, the Board of Supervisors is being asked to authorize the Director of the Human Services Agency or the Director’s designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions; and
WHEREAS, this Board has been presented with the form of the Second Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into said Amendment.
NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that
A. The President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and
B. The Director of the Human Services Agency or the Director’s designee shall be authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
