RESOLUTION A) WAIVING THE REQUEST FOR PROPOSAL PROCESS; AND; B) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH DOCUMENT FULFILLMENT SERVICES (DFS) TO PROVIDE FOR THE PICKUP OF DVD DATA DISKS, PRINTING, FOLDING, STUFFING, AND MAILING OF CLIENT CORRESPONDENCE GENERATED BY THE SOFTWARE SYSTEM RESULTING FROM THE COUNTY OF SAN MATEO HUMAN SERVICES AGENCY’S MIGRATION TO THE CALWORKS INFORMATION NETWORK (CALWIN), EXTENDING THE TERM BY TWELVE (12) MONTHS TO JUNE 30, 2010, INCREASING THE AMOUNT BY $170,000 TO $565,000, NO NET COUNTY COST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, it is in the County’s best interest to waive the Request for Proposals (RFP) process to prevent a service delay, and HSA plans to conduct a RFP in 2010; and

WHEREAS, on December 12, 2006, the Board of Supervisors approved by Resolution No. 068442 an Agreement with the Document Fulfillment Services in the amount of $225,000, for the term of January 1, 2007 through June 30, 2008, to provide for the pickup of DVD data disks, printing, folding, stuffing, and mailing of client correspondence generated by the software system resulting from the County of San Mateo Human Services Agency’s migration to the CalWORKs Information Network (CalWIN); and

WHEREAS, on June 24, 2008, the Board of Supervisors adopted by Resolution No. 069539 authorizing an Amendment to the Agreement with the Document Fulfillment Services, increasing the amount by $170,000, for a total obligation of $395,000, and extended the term for an additional year to June 30, 2009; and

WHEREAS, the parties wish to further amend the Document Fulfillment Services Agreement, increasing the amount by $170,000, for a total obligation of $565,000, to continue to provide for CalWIN pickup of DVD data disks, printing and mailing services, and extend the term for an additional year to June 30, 2010; and

WHEREAS, this Board has been presented with a form of such Amendment, and said Board has examined and approved it as to both form and content and desires to enter into same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that

A) The Request for Proposal (RFP) Process is waived; and

B) The President of this Board of Supervisors is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
