RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT NO. 3 TO THE AGREEMENT WITH MAXONIC MILESTONE MANAGEMENT FOR CONSULTING SERVICES TO EXTEND THE TERM END DATE TO DECEMBER 31, 2009, AND ADD UP TO $90,000 FOR A NEW TOTAL MAXIMUM FISCAL OBLIGATION OF $228,000.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on October 12, 2008, the Information Services Department entered into an Agreement in the amount of $25,000 for the term of October 12, 2008, to June 30, 2009, with Maxonic Milestone Management to provide the Information Services Department with professional services relating to the County’s Internet and Intranet;

WHEREAS, on December 4, 2008, the County Manager approved a Change Order to the Agreement to increase the amount by up to $50,000;

WHEREAS, on February 10, 2009, this Board waived the Request for Proposal process and approved a Amendment to the Agreement to increase the amount by up to $63,000;

WHEREAS, this Board having considered the matter has determined that it would be in the best interest of the County to approve an Amendment to the Agreement with Maxonic Milestone Management to increase the contract amount by up to $90,000, for a new total fiscal obligation not to exceed $228,000 and extend the term end date to December 31, 2009;

WHEREAS, this Board has been presented with a form of the Amendment, has examined and approved it as to both form and content, and desires to enter into same;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
