Exhibit “A”

In consideration of the payments set forth in Exhibit “B”, Contractor shall provide the following services:

I. Description of Services to be Performed by the Contractor

A. Contractor will provide janitorial and maintenance services, five days per week at the Fair Oaks Adult Clinic located at 2710 Middlefield Road, Redwood City, CA 94063; at the Fair Oaks Children’s Health Center located at 630 Laurel St., Ste. B., Redwood City; at the Methadone Clinic located at 795 Willow Rd., Building 137, Menlo Park (VA campus); at the Sequoia Redwood City Teen Wellness Center located at 200 James Avenue, Redwood City, CA 94063; at the Willow Clinic, 795 Willow Rd., Building 334, Menlo Park, CA 94063; and beginning October 1, 2008, at the South San Francisco Clinic located at 206 Spruce St., South San Francisco, CA 94080. Contractor Management will conduct monthly service reviews to ensure that all required services are performed to the satisfaction of the Clinic Manager.

I. Janitorial and maintenance will include the following scope of services:

a. Areas to be services: Frequency

All Entrance/Reception/Waiting Areas	5 times per week
All Exam/Procedure/Counseling Rooms	5 times per week
All Offices and Screening Rooms	5 times per week
All Records/File Rooms	5 times per week
All Nurses Stations	5 times per week
All Kitchenette/Break/Lunchroom Areas	5 times per week
All Restrooms	5 times per week
All Hallways and Common Areas	5 times per week
All Lab/Treatment Dispensing Areas	5 times per week
All General Clinic Areas	5 times per week

b. Basic Services: Frequency

Police front of all buildings for debris and sweep as needed (Except at Willow Clinic)	5 times per week
Police and pick up debris in parking lots (Except at Willow Clinic)	As needed
Clean and wipe all ash-urns in entry ways	As needed
Dust P.C.s (Do Not Touch Keyboards)	1 time per month
Clean and Mop all Exam Rooms (Not Exam Tables)	5 times per week
Clean and disinfect drinking fountains and water dispenser	As needed
Clean and straighten all Reception counters and Patient waiting areas	5 times per week
Wipe and/or dust all open flat spaces (do not move papers or personal items)	As needed
Empty all waste receptacles, install fresh liner if needed (Place in designated location)	5 times per week
Remove recycling, place in proper bins	As needed
Wipe and dust top of partitions	2 times per week
Clean all window sills and ledges	1 time per week
High dusting: inspect for cobwebs and clean	2 times per month
Vacuum air conditioning and heating grills	4 times per year
Spot clean doors, door frames and light switches	As needed
Dust file cabinets, chairs, and other office furniture	As needed

c. All Break/Lunchroom/Coffee Areas Frequency

Clean and wipe all cabinet faces and counter tops	5 times per week
Clean and wipe all tables, chairs, and appliance surfaces	5 times per week
Clean and disinfect sink	5 times per week
Restock any paper disposables	5 times per week
Wipe down interior and exterior of microwave	5 times per week
Clean and wipe trash receptacle tops/lids	5 times per week
Clean out interior of refrigerator – SSF and Willow Clinics Only (Put advance notice on refrigerator door for employees)	1 time per week

d. All Restrooms: Frequency

Clean and sanitize doors and partitions	5 times per week
Wipe counter tops, dispenser, toilets	5 times per week
Sanitize door handles and light switches	5 times per week
Clean and disinfect toilets and urinals (including base and rear)	5 times per week
Polish all chrome fixtures and stainless steel	5 times per week
Clean all mirrors	5 times per week
Dispose of waste and install new liners	5 times per week
Spot clean and wipe waste receptacles	5 times per week
Sweep and wet mop floors using hospital approved disinfectant cleaner	5 times per week
Restock handsoaps and all paper disposables	5 times per week
Wipe all partition tops	5 times per week

e. Carpets: Frequency

Spot vacuum all carpeted areas	5 times per week
Vacuum and clean all mats	5 times per week
Remove all spots and spills	5 times per week

f. Hard Surface Floors: Frequency

Sweep and/or dust mop all (a) vinyl, (b) ceramic, (c) granite/marble floors	5 times per week
Thoroughly mop all floors	5 times per week
Buff and High Speed Polish designated clinic VCT floor	1 x per month

g. Windows and Glass: Frequency

Clean all doors in entrance ways, inside and out	5 times per week
Dust and/or vacuum blinds	4 times per year
Spot clean partition glass	As needed

h. Security and Communication: Frequency

Make sure all windows and doors are locked	5 times per week
Disarm and rearm security system	5 times per week
Report all out of order items to proper personnel	As needed

i. Materials and Equipment

Contractor will provide all cleaning chemicals and equipment

County will provide restroom products and liners

j. Utility Work – South San Francisco Clinic: Frequency

Buff floors	1 time per month
Vacuum room vents, including bathroom vents	1 time per month
Clean, sweep and mop building lobby area	5 times per week
Clean and polish elevator inside and out	5 times per week
Vacuum hallway stairs	5 times per week

Exhibit “B”

In consideration of the services provided by Contractor in Exhibit “A,” County shall pay Contractor based on the following fee schedule:
