RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A THIRD AMENDMENT TO THE AGREEMENT WITH PACIFIC RIM BUILDING SERVICES TO PROVIDE JANITORIAL SERVICES, FIVE DAYS PER WEEK, AT THE FAIR OAKS ADULT CLINIC, THE FAIR OAKS CHILDREN’S CLINIC, THE METHADONE CLINIC, THE SEQUOIA TEEN WELLNESS CENTER, THE WILLOW CLINIC AND THE SOUTH SAN FRANCISCO CLINIC, ENHANCING SERVICES AND INCREASING THE MAXIMUM AMOUNT PAYABLE BY $120,000 FROM $255,010 TO $375,010, WITH NO CHANGE IN TERM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on July 10, 2007, by Resolution 068862, this Board approved an Agreement with Pacific Rim Building Services to provide janitorial services, five days per week, at the Fair Oaks Adult Clinic, the Fair Oaks Children’s Clinic, the Methadone Clinic and the Sequoia Teen Wellness Center; and

WHEREAS, on December 27, 2007, the Chief Executive Officer of San Mateo Medical Center (SMMC) approved an Amendment to the Agreement which added the Willow Clinic and increased funding; and

WHEREAS, On July 8, 2008, by Resolution 069564, this Board approved a second Amendment to the Agreement which added Willow Clinic for the remainder of the contract term, added South San Francisco Clinic for 18 months, and increased the maximum amount of the Agreement, and extended the term of the Agreement; and

WHEREAS, the Fair Oaks Children’s Clinic has been renovated, increasing the size, the Sequoia Teen Wellness Center has relocated from a mobile trailer to a building, and additional services have been requested by the Clinic Managers at both Willow and South San Francisco Clinics; and

WHEREAS, this Board has been presented with a third Amendment to the Agreement with Pacific Rim Building Services for these services and has approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute the Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
