RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO 1) EXECUTE A GRANT AGREEMENT FROM BLUE SHIELD OF CALIFORNIA FOUNDATION FOR THE TERM OF JUNE 1, 2009 TO DECEMBER 1, 2011, IN THE AMOUNT OF $200,000 TO EXTEND AND BROADEN THE EVALUATION OF THE BLUE RIBBON TASK FORCE PILOT; AND 2) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH URBAN INSTITUTE FOR EVALUATION SERVICES, TO EXTEND THE TERM TO DECEMBER 31, 2011, INCREASE THE AMOUNT BY $200,000 FOR A NEW TOTAL OF $542,000, AND MODIFY THE SCOPE TO INCLUDE ADDITIONAL EVALUATION ACTIVITIES.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
|
WHEREAS, the Blue Ribbon Task Force for Adult Health Care Expansion (Task Force) created the San Mateo Access and Care for Everyone (ACE) program through a grant from the State of California Department of Health Care Services; and
|
|
WHEREAS, this Board accepted a grant from the California Endowment in the amount of $345,750 to fund an evaluation of the ACE program on January 8, 2008; and
|
|
WHEREAS, this Board approved an Agreement with the Urban Institute (UI) in the amount of $342,000 to perform the evaluation on July 8, 2008; and
|
|
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Blue Shield of California Foundation (BSCF) shall provide funding to the San Mateo County Health System, on behalf of the County of San Mateo, to support an extension of the evaluation of the ACE Program; and

WHEREAS, the term of the Grant Agreement with BSCF is June 1, 2009 to December 1, 2011, for a total amount of $200,000; and
|
|
WHEREAS, the grant funds will be used to increase the Agreement with UI, extend its term and include additional evaluation activities; and
|
|
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to Agreement, reference to which is hereby made for further particulars, whereby the term of the Agreement with UI will be extended to December 31, 2011, the amount will be increased by $200,000 for a new total of $542,000 and the scope of services will be modified; and

WHEREAS, this Board of Supervisors has been presented with a form of the BSCF Agreement and has examined and approved the Agreement as to form and content and desires to enter into the Agreement; and

|
WHEREAS, this Board of Supervisors has been presented with a form of the UI Amendment to Agreement and has examined and approved the Amendment as to form and content and desires to approve this Amendment to the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Grant Agreement with Blue Shield of California Foundation and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FUTHER RESOLVED that the President of this Board be and is hereby authorized and directed to execute the Amendment to Agreement with Urban Institute for and on behalf of the County of San Mateo Health System, and the Clerk of the Board shall attest the President’s signature thereto.
