RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT ONE TO THE AGREEMENT WITH BREATHE CALIFORNIA: GOLDEN GATE PUBLIC HEALTH PARTNERSHIP TO PROVIDE TOBACCO CESSATION SERVICES AND A PUBLIC EDUCATION CAMPAIGN ON THE IMPACT OF TOBACCO USE IN FILMS, EXTENDING THE TERM FROM SEPTEMBER 30, 2009 THROUGH JUNE 30, 2010, AND INCREASING THE AGREEMENT MAXIMUM BY $80,000 FROM $132,000 TO A NEW MAXIMUM OF $212,000; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the parties entered into an Agreement on August 29, 2008, to provide tobacco cessation services for the term on July 1, 2008 through June 30, 2009, for a maximum amount of $80,000; and

 

WHEREAS, in January 2009 the parties entered into an Agreement that superseded the Agreement signed on August 29, 2008, extended the term through September 30, 2009, increased the maximum obligation by $52,000 to $132,000, and added the service to execute a public campaign to educate the public regarding the harm to youth of exposure to tobacco use in film media; and

 

WHEREAS, the parties now wish to amend the Agreement to extend the term through June 30, 2010, and increase the amount by $80,000 increasing the Agreement maximum obligation from $132,000 to $212,000.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute Amendment One to the Agreement for and behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
