BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A GRANT AGREEMENT WITH FIRST 5 SAN MATEO COUNTY TO SUPPORT THE PRENATAL TO THREE INITIATIVE FOR THE TERM OF JULY 1, 2009 THROUGH JUNE 30, 2012, IN THE AMOUNT OF $3,152,000; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Grant Agreement with First 5 San Mateo County, reference to which is hereby made for further particulars, whereby First 5 San Mateo County will provide funding for the Health System’s Prenatal to Three Initiative in the amount of $3,152,000 for the term of July 1, 2009 through June 30, 2012; and

WHEREAS, this Board has been presented with a form of the Grant Agreement and has examined and approved it as to both form and content and desires to enter into said Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term of services is/are within the current or revised fiscal provisions.
