RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A FIFTH AMENDMENT TO THE AGREEMENT WITH THE STATE OF CALIFORNIA MANAGED RISK MEDICAL INSURANCE BOARD TO: ESTABLISH NEW ADMINISTRATIVE POLICIES IN EXHIBITS A AND B OF THE AGREEMENT; EXTEND THE TERM OF THE AGREEMENT FROM JULY 1, 2005 THROUGH JUNE 30, 2009 TO JULY 1, 2005 THROUGH JUNE 30, 2010; AND INCREASE THE CONTRACT AMOUNT BY $309,789, FOR A TOTAL OBLIGATION OF $2,043,014

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, in January 2006 this Board of Supervisors adopted Resolution 67753, approving an Agreement with the State of California’s Managed Risk Medical Insurance Board to receive federal matching funds for the Children’s Health Initiative Program; and

WHEREAS, in June 2007 this Board of Supervisors adopted Resolution 68825 approving a new rate not to exceed $165.63 per-member per-month for a child less than one year of age and $75.64 per-member per-month for a child one through eighteen years of age and authorizing the Health System Chief to execute contract amendments as long as the modified services are within the current or revised fiscal provisions; and

WHEREAS, in February 2008 this Board of Supervisors adopted Resolution 69285 extending the term of the Agreement to July 1, 2005 through June 30, 2008, approving a new rate not to exceed $75.67 per-member per-month for a child one through eighteen years of age, and establishing new administrative policies in Exhibit B and D of the Agreement ; and

WHEREAS, in August 2008 this Board of Supervisors adopted Resolution 69632 extending the term of the Agreement to July 1, 2005 through June 30, 2009, and establishing new administrative policies in Exhibit A and B of the Agreement; and

WHEREAS, in January 2009 the Health Systems Chief approved the Fourth Amendment to this Agreement decreasing the rates to $157.43 per-member per-month for a child less than one year of age and $71.92 per-member per-month for a child one through eighteen years of age; and

WHEREAS, both parties now wish to extend the term of the Agreement to July 1, 2005 through June 30, 2010, increase the funding amount by $309,789 for a new maximum of $2,043,014; and establish new administrative policies in Exhibits A and B of the Agreement, and

WHEREAS, this Board of Supervisors has been presented with the Amendment to Agreement and has examined and approved the Amendment as to form and content and desires to approve this Amendment to the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board be and is hereby authorized and directed to execute the Amendment to Agreement with the State of California’s Managed Risk Medical Insurance Board for and on behalf of the County of San Mateo Health System, and the Clerk of the Board shall attest the President’s signature thereto.
