RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH FRONT STREET, INC. FOR THE PROVISION OF BOARD AND CARE SERVICES FOR ADULTS, EXTENDING THE TERM THROUGH JUNE 30, 2011, AND INCREASING THE MAXIMUM OBLIGATION BY $1,253,175 TO A NEW MAXIMUM OF $1,533,850

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on January 8, 2009, the Interim Chief of the Health System approved an Agreement with Front Street, Inc., by Agreement Number 61000-09-D018, for the provision of board and care services for older adults with serious and persistent mental illness, for the term January 1, 2009 through March 31, 2009, for a maximum obligation of $25,000; and

 

WHEREAS, on March 3, 2009, this Board of Supervisors approved an amendment to the Agreement with Front Street, Inc., by Resolution Number 069950, for the provision of board and care services for older adults with serious and persistent mental illness, for the term January 1, 2009 through June 30, 2010, for a maximum obligation of $280,675; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Second Amendment to the Agreement, reference to which is hereby made for further particulars, whereby the maximum obligation is being increased by $1,253,175 to a new maximum obligation of $1,533,850, and the term of the Agreement is being extended through June 30, 2011; and

 

WHEREAS, the Board of Supervisors has examined and approved the Amendment to the Agreement as to form and content and desires to enter into same.

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
