RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH LABORATORY CORPORATION OF AMERICA FOR THE PROVISION OF CLINICAL LABORATORY SERVICES, EXTENDING THE TERM THROUGH JANUARY 31, 2010, AND INCREASING THE MAXIMUM OBLIGATION BY $122,500 TO A NEW MAXIMUM OF $332,500.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 8, 2008, this Board approved an Agreement with Laboratory Corporation of America for the provision of clinical laboratory services for clients of Behavioral Health and Recovery Services and Correctional Health Services for the term July 1, 2008 through June 30, 2009, for a maximum obligation of $210,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to that Agreement, reference to which is hereby made for further particulars, whereby the term is extended through January 31, 2010, and the maximum obligation is increased by $122,500 to a new maximum of $332,500; and

WHEREAS, this Board has examined the Amendment to the Agreement and has approved it as to both form and content and desires to enter into the Amendment to the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
