RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE THE AGREEMENTS WITH ALZHEIMER’S ASSOCIATION, NORTHERN CALIFORNIA & NORTHERN NEVADA; AVENIDAS; CATHOLIC CHARITIES CYO; CENTER FOR INDEPENDENCE OF THE DISABLED; CITY OF BELMONT; CITY OF DALY CITY; CITY OF EAST PALO ALTO; CITY OF MENLO PARK; CITY OF PACIFICA; CITY OF SAN BRUNO; CITY OF SAN MATEO; CITY OF SOUTH SAN FRANCISCO; COASTSIDE ADULT DAY HEALTH CENTER; EDGEWOOD CENTER FOR CHILDREN AND FAMILIES; FAMILY CAREGIVER ALLIANCE; FAMILY SERVICE AGENCY; HOSPITAL CONSORTIUM OF SAN MATEO COUNTY; KIMOCHI, INC.; LEGAL AID SOCIETY OF SAN MATEO COUNTY; MILLS-PENINSULA HEALTH SERVICES; OMBUDSMAN SERVICES OF SAN MATEO COUNTY, INC.; PENINSULA VOLUNTEERS, INC.; SECOND HARVEST FOOD BANK; SELF HELP FOR THE ELDERLY; AND SENIOR COASTSIDERS FOR THE PROVISION OF SUPPORTIVE, NUTRITION, AND COMMUNITY-BASED SERVICES FOR THE TERM OF JULY 1, 2009 THROUGH JUNE 30, 2010, FOR THE TOTAL AMOUNT OF $3,151,236; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE FUNDING AMOUNT BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs pursuant to the Older Americans Act and the Older Californians Act; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, Agreements between the County of San Mateo and Alzheimer’s Association, Northern California & Northern Nevada; Avenidas; Catholic Charities CYO; Center for Independence of the Disabled; City of Belmont; City of Daly City; City of East Palo Alto; City of Menlo Park; City of Pacifica; City of San Bruno; City of San Mateo; City of South San Francisco; Coastside Adult Day Health Center; Edgewood Center for Children and Families; Family Caregiver Alliance; Family Service Agency; Hospital Consortium of San Mateo County; Kimochi, Inc.; Legal Aid Society of San Mateo County; Mills-Peninsula Health Services; Ombudsman Services of San Mateo County, Inc.; Peninsula Volunteers, Inc.; Second Harvest Food Bank; Self-Help for the Elderly; and Senior Coastsiders, references to which are hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same; and

WHEREAS, all parties wish to enter into the Agreements to provide Adult Day Care/Adult Day Health Care, Alzheimer’s Program, Brown Bag Program, Case Management, Congregate Nutrition, Caregiver Support, Health Insurance Counseling and Advocacy Program, Health Promotion/Disease Prevention, Home-Delivered Meals, Legal Assistance, Ombudsman Services, Senior Employment, and Transportation services to older adults and adults with disabilities in the amount of $3,151,236 for the term of July 1, 2009 through June 30, 2010.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee shall be authorized to execute contract amendments which modify the funding amount by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
