RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT NO. 3 TO THE AGREEMENT WITH PICIS, INC. TO PROVIDE SUPPORT AND MAINTENANCE OF THE EMERGENCY DEPARTMENT INFORMATION SYSTEM (EDIS) AT THE SAN MATEO MEDICAL CENTER (SMMC), INCREASING THE CONTRACT AMOUNT BY UP TO $192,501 FOR A NEW TOTAL MAXIMUM FISCAL OBLIGATION OF $845,001 AND EXTENDING THE TERM THROUGH JUNE 30, 2012.
RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
WHEREAS, in December 2005, the County and Picis, Inc. entered into an Agreement for the implementation of an Emergency Department Information System for the San Mateo Medical Center’s Emergency Department;
WHEREAS, Picis, Inc. and the County of San Mateo now wish to amend the Agreement for the San Mateo Medical Center’s EDIS to provide support and maintenance, increasing the contract amount by up to $192,501 for a new total maximum fiscal obligation of $845,001 and extending the term through June 30, 2012;
WHEREAS, this Board has been presented with a form of the Amendment, said Board has examined and approved same as to both form and content and desires to enter into same.
NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors is authorized and directed to execute said Amendment No. 3 as is approved by the County Counsel for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
