BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

    RESOLUTION A)AUTHORIZING AN INCREASE OF $7,000.00 IN THE AMOUNT APPROVED FOR THE PURCHASE OF A 64 SLICE CT SCANNER FOR AN AMOUNT NOT TO EXCEED $929,000; AND B) AUTHORIZING AN EXTENSION IN THE TERM OF THE PURCHASE ORDER THROUGH DECEMBER 31, 2009; AND C) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERMS AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on April 8, 2008, this Board approved resolution 069354 authorizing the purchase and installation of a CT Scanner system for an amount not to exceed $922,000; and

 

WHEREAS, due to an intervening increase in the sales tax rate applicable to the purchase of the CT Scanner system, an additional $7,000 is needed to pay for the CT Scanner system; and

 

WHEREAS, it appears appropriate to extend the terms of this purchase order

for the purchase of the CT Scanner and its components to December 31, 2009: and

 

WHEREAS, this Board has been presented with a form of amendment to the

purchase order which addresses the matters set forth above; and

 

WHEREAS, this Board has considered the Amendment, has approved it as to both form and content, and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute the Amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee shall be authorized to execute contract amendments which modify the county’s maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
