RESOLUTION A) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE SECOND AMENDMENT TO THE AGREEMENT WITH FULL CAPACITY MARKETING, INC TO PROVIDE SOCIAL MARKETING SERVICES. THE TERM REMAINS THE SAME, SEPTEMBER 24, 2009 TO SEPTEMBER 30, 2010, AND THE AMOUNT IS INCREASED BY $115,000 TO A TOTAL AMOUNT OF $214,800 IN WHICH $3,750 IS NET COUNTY COST; AND B) AUTHORIZING THE DIRECTOR OF HUMAN SERVICES AGENCY OR THE DIRECTOR'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the parties entered into Agreement No. 73200-10-C117 on September 24, 2009 for providing social marketing services in the amount of $74,000 for the term of September 24, 2009 to September 30, 2010; and

WHEREAS, the parties entered into a First Amendment on November 3, 2009 to increase the amount by $25,800 for a total obligation of $99,800 and the term remained the same; and

WHEREAS, the parties wish to amend the Agreement to increase the amount by $115,000 for a total obligation of $214,800, for purposes of marketing services related to the American Recovery and Reinvestment Act (ARRA) and the term remains the same; and

WHEREAS, this Board has been presented with a form of such Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that:

A) The President of this Board of Supervisors is hereby authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and

B) The Director of Human Services Agency or the Director’s Designee is authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
