RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH EDGEWOOD CENTER FOR CHILDREN AND FAMILIES FOR COMPREHENSIVE FULL SERVICE PARTNERSHIP SERVICES TO ADD INTEGRATED FULL SERVICE PARTNERSHIP SERVICES AND INCREASE THE MAXIMUM OBLIGATION BY $3,142,684 TO A NEW MAXIMUM OF $12,396,148, WITH NO CHANGE TO THE TERM OF JULY 1, 2009 T0 JUNE 30, 2012

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby Edgewood Center for Children and Families shall provide comprehensive (effective July 1, 2009) and integrated (effective February 1, 2010) Full Service Partnership (FSP) services for children, youth and transition-age youth (TAY), Therapeutic Behavioral Services (TBS) for FSP enrollees, and administration of non-FSP flexible funds during the Agreement term of July 1, 2009 to June 30, 2012, for an increase of $3,142,684 to a new maximum obligation of $12,396,148; and

WHEREAS, this Board of Supervisors has been presented with the Amendment to the Agreement and has approved it as to both form and content and desires to enter into the Amended Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
