RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH BLOOD CENTERS OF THE PACIFIC INCREASING THE MAXIMUM AMOUNT PAYABLE BY $712,100 TO $1,424,200, EXTENDING THE TERM OF THE AGREEMENT BY ONE YEAR FOR THE TERM JANUARY 1, 2009 THROUGH DECEMBER 31, 2010, AND INSTITUTING A QUARTERLY REBATE SYSTEM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, On January 6, 2009, the Board authorized an Agreement with Blood Centers of the Pacific (Blood Centers) to provide blood and blood products to San Mateo Medical Center (SMMC), for the term January 1, 2009 to December 31, 2009, in an amount not to exceed $712,100; and

WHEREAS, SMMC continues to require blood and blood products in order to provide care to its medically indigent adult (MIA) population; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, an Amendment to the Agreement with Blood Centers, reference to which is hereby made for further particulars, whereby the term of the Agreement is extended by one year, to December 31, 2010, the maximum amount payable thereunder is increased by $712,100, to $1,424,200, and a quarterly rebate system is instituted; and

WHEREAS, the Board has considered the Amendment to the Agreement and approved it as to both form and content and desires to enter into it.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
