Agreement with Shelter Network

 

RECOMMENDATION:

Adopt a Resolution:

1.

Authorizing the President of the Board to execute an Agreement with Shelter Network to provide case management services to San Mateo County’s homeless population for the term of November 1, 2009 through October 31, 2010, in an amount not to exceed $221,075; and

   

2.

Authorizing the Chief of the Health System or designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term and services is/are within the current or revised fiscal provisions.

   

BACKGROUND:

San Mateo Medical Center (SMMC) was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Health Care of the Health and Resources Services Administration to provide primary, dental, and behavioral health care to San Mateo County’s homeless population. The HCH Program is designed to assist homeless adults, families and youth with access to and utilization of SMMC’s continuum of care and other support services. The HCH Program contracts with both County and community-based programs to provide this care.

After implementing a planning process and conducting a Strengths, Weaknesses, Opportunities and Threats (SWOT) analysis in 2007, a Request for Proposals (RFP) was released to address the program’s mission and goals. Shelter Network’s proposal to provide case management services was one of the selected proposals to become HCH contractors.

 

DISCUSSION:

Shelter Network was selected to receive HCH funding to provide comprehensive case management services to homeless individuals residing at Maple Street and Safe Harbor shelters and unsheltered homeless individuals residing on the street. Under the Agreement Shelter Network will provide initial assessments and on-going case management services to 925 unique homeless individuals. This Agreement was delayed because the goals and objectives of this Agreement were significantly revised to better emphasize increased access to SMMC’s primary care services to homeless residents and to focus intensive case management efforts towards shelter residents experiencing chronic health conditions.

The contractor has assured compliance with the County’s Contractor Employee Jury Service Ordinance, as well as all other contract provisions that are required by County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.

 

Risk Management has reviewed and approved the Agreement. County Counsel has reviewed and approved the Resolution and Agreement as to form.

 

Approval of this Agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing access to case management services to homeless individuals residing at shelters and to unsheltered homeless individuals residing on the streets of San Mateo County. It is anticipated that 75% of these individuals will access primary care through the San Mateo County Health System. By accessing primary care and receiving ongoing case management services these individuals will experience an improvement in their overall health.

FISCAL IMPACT:

The term of the Agreement is November 1, 2009 through October 31, 2010. The maximum amount payable under the Agreement is $221,075. All funds for this Agreement are provided by the HCH grant. Expenditures and grant revenue of $50,000 for this Agreement are included in the SMMC FY 2009-10 Adopted Budget. Grant revenue in the amount of $171,075 will be included in the SMMC FY 2010-11 Recommended Budget. There is no Net County Cost.
