RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A THIRD AMENDMENT TO THE AGREEMENT WITH YOUTH AND FAMILY ENRICHMENT SERVICES (YFES) FOR THE PURPOSE OF PROVIDING ADDITIONAL YOUTH WITH COMMUNITY-BASED PREVENTION SERVICES THROUGH THE TRANSITIONAL HOUSING PLACEMENT PLUS (THP+) PROGRAM, THE TERM REMAINS JULY 1, 2007 THROUGH JUNE 30, 2010, INCREASING THE AMOUNT BY $387,000 TO $2,640,890, THERE IS NO NET COUNTY COST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County and Contractor entered into an Agreement by Resolution Number 069098 for the purpose of providing case management services for Your House Residential Services, Crisis Intervention Suicide Prevention Services, Aftercare Services, Transitional Housing Placement Program (THPP), the Daybreak Program, as well as counseling and residential services, in the amount of $1,042,537 and term of July 1, 2007 to June 30, 2009; and

WHEREAS, this Agreement has been amended on two occasions, first on December 16, 2008 by Resolution Number 069845 and again on June 23, 2009 by Resolution Number 070248 increasing the amount by a total of $1,211,353 for additional services to Transitional Housing Placement Plus (THP+), adding case management services and to add information on the referral and discharging process for THPP youth in FY 2008-09 and FY 2009-10. Additionally, the term was extended by one year to June 30, 2010; and

WHEREAS, the parties wish to further amend the Agreement for the purpose of increasing the amount by $387,000 for FY 2009-10 for a total obligation of $2,640,890 for purpose of serving additional youth for community-based prevention services for the Transitional Housing Placement Plus (THP+) Program and the term remains the same; and

WHEREAS, this Board has been presented with the form of the Third Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that

the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Third Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
