BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH SAN MATEO HEALTH COMMISSION FOR THE PROVISION OF MEDI-CAL SERVICES FOR HEALTH PLAN OF SAN MATEO CLIENTS WHO ARE RESIDING IN SAN MATEO COUNTY LONG TERM CARE FACILITIES FOR THE TERM FEBRUARY 1, 2010 THROUGH JANUARY 31, 2011; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, San Mateo Health Commission dba Health Plan of San Mateo (HPSM) and the County seek to enter into an Agreement whereby HPSM will reimburse the County for providing services for patients who are HPSM members and residents of the County’s Long Term Care facilities; and

 

WHEREAS, both parties now wish to enter into an Agreement, whereby County Long Term Care facilities will provide care for Medi-Cal residents who are clients of HPSM for the period February 1, 2010 through January 31, 2011; and

 

WHEREAS, this Board has been presented with a form of such Agreement and said Board has examined and approved same as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
