BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING (A) THE PRESIDENT OF THE BOARD TO EXECUTE A REVENUE AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF REHABILITATION FOR TRAINING AND EMPLOYMENT SERVICES, THE TERM IS JULY 1, 2010 THROUGH JUNE 30, 2013, FOR THE AMOUNT OF $855,414, THERE IS NO NET COUNTY COST; AND (B) AUTHORIZING THE DIRECTOR OF THE HUMAN SERVICES AGENCY (HSA) OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, HSA and the State of California, DOR have joined in partnership and developed an Agreement for the vocational rehabilitation of consumers with disabilities in San Mateo County; and

 

WHEREAS, pursuant to the Agreement, HSA will provide vocational assessment, employment preparation, job development and placement services for populations with significant physical and psychiatric disabilities not receiving services from County Mental Health and the State of California will provide funds to HSA with respect to the same; and

 

WHEREAS, DOR has allocated the amount of $855,414 to the County over a three year term of July 1, 2010 through June 30, 2013; and

 

WHEREAS, this Board has been presented with a form of such Agreement, and said Board has examined and approved same as to both form and content and desires to enter into the same.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that:

A. The President of this Board of Supervisors is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President's signature thereto; and

B. The Director of the Human Services Agency or the Director’s designee is hereby authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provision.
