BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING (A) THE PRESIDENT OF THE BOARD TO EXECUTE A REVENUE AGREEMENT WITH TELECARE TRANSITIONS IN WHICH THE COUNTY’S CATERING CONNECTION WILL CATER EVENING MEALS FOR THE TERM OF MARCH 1, 2010 TO FEBRUARY 28, 2013, IN THE AMOUNT OF UP TO $300,000, THERE IS NO NET COUNTY COST AND; (B) THE DIRECTOR OF THE HUMAN SERVICES AGENCY (HSA) OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, Catering Connection was awarded this Revenue Agreement in the amount of $300,000 over a three year term of March 1, 2010 to February 28, 2013 to provide catering services to the residents of Telecare Transitions;

WHEREAS, this Board has been presented with a form of such Agreement and said Board has examined and approved same as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that:

A. The President of this Board of Supervisors is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President's signature thereto; and

B. The Director of the Human Services Agency or the Director's designee is hereby authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions..
