RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A FIRST AMENDMENT TO THE AGREEMENT WITH DALY CITY PENINSULA PARTNERSHIP COLLABORATIVE FOR THE IMPLEMENTATION OF DRUG PREVENTION AND MENTAL HEALTH SERVICES, INCREASING THE MAXIMUM OBLIGATION BY $201,000, FOR A NEW MAXIMUM OF $226,000, AND EXTENDING THE TERM TO JANUARY 1, 2010 THROUGH JUNE 30, 2011; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on February 2, 2010, the Chief of the Health System approved an Agreement with Daly City Peninsula Partnership Collaborative (DCPPC) for the provision of alcohol and other drug prevention services, for the term January 1, 2010 through June 30, 2010, for a maximum obligation of $25,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby the Daly City Peninsula Partnership Collaborative shall provide implementation of drug prevention and mental health services during the Agreement term of January 1, 2010 through June 30, 2011, for a revised maximum obligation of $226,000; and

WHEREAS, this Board has examined the Amendment to the Agreement and has approved it as to both form and content and desires to enter into the Amendment.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
