BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AGREEMENTS WITH ADDUS HEALTHCARE, INC.; ARCADIA HEALTH SERVICES, INC., DOING BUSINESS AS ARCADIA HEALTH CARE; GOOD SHEPHERD HOLDINGS CORP., DOING BUSINESS AS BRIGHTSTAR CARE OF SAN MATEO; MAXIM HEALTHCARE SERVICES; MEDICAL CARE PROFESSIONALS; AND NP PLUS, LLC, DOING BUSINESS AS PROFESSIONAL HEALTHCARE AT HOME FOR HOME HEALTH AND ATTENDANT CARE SERVICES FOR THE TERM JULY 1, 2010 THROUGH JUNE 30, 2013, IN THE COLLECTIVE AMOUNT OF $970,000; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), FOR EACH AGREEMENT, AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Agreements, references to which are hereby made for further particulars, whereby Addus HealthCare, Inc.; Arcadia Health Services, Inc., doing business as Arcadia Health Care; Good Shepherd Holdings Corp., doing business as BrightStar Care of San Mateo; Maxim Healthcare Services; Medical Care Professionals; and NP Plus, LLC., doing business as Professional Healthcare at Home shall provide home health and attendant care services for the Centralized Intake/Adult Protective Services, Long-Term Supportive Services Program, Multipurpose Senior Services Program, and the Public Guardian program; and

 

WHEREAS, all parties now wish to enter into Agreements to provide home health and attendant care services for the period of July 1, 2010 to June 30, 2013, for a total obligation of $970,000 collectively; and

 

WHEREAS, this Board has examined the Agreements and has approved them as to both form and content.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), for each Agreement, and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
