RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH THE MENTAL HEALTH ASSOCIATION OF SAN MATEO COUNTY FOR THE PROVISION OF TRANSITIONAL RESIDENTIAL TREATMENT AND SUPPORTED HOUSING FOR SERIOUSLY MENTALLY ILL ADULTS, HOUSING FOR TRANSITION-AGE YOUTH AND FLEXIBLE FUND ADMINISTRATION, FOR THE PERIOD JULY 1, 2010 THROUGH JUNE 30, 2011, FOR A MAXIMUM OBLIGATION OF $1,582,283; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

 
 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California

 

WHEREAS, there has been presented to this Board of Supervisors an Agreement whereby the Mental Health Association of San Mateo County shall provide transitional residential treatment and supported housing for seriously mentally ill adults with mental health and co-occurring disorders who are at risk of homelessness, supported housing for full-service partnership transition-age youth enrollees, and Flexible Fund administration for Adults and Older Adult Services and the Pathways Program for the period July 1, 2010 through June 30, 2011, for a maximum obligation of $1,582,283; and

 

WHEREAS, this Board has approved the Agreement as to both form and content and desires to enter into the Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED, that the Chief of the Health System or designee be and is hereby authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and /or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
