BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH YOUTH & FAMILY ENRICHMENT SERVICES FOR THE PROVISION OF PSYCHIATRIC SERVICES INCREASING THE MAXIMUM OBLIGATION BY $75,000 TO A NEW MAXIMUM OF $1,294,924 WITH NO CHANGE TO THE OF TERM JULY 1, 2009 THROUGH JUNE 30, 2010; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM AND DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on July 7, 2009, this Board of Supervisors approved an Agreement with Youth and Family Enrichment Services for the period July 1, 2009 through June 30, 2010 for the provision of mental health services, support for the Youth Development Initiative, community worker services and tobacco cessation services for a maximum obligation of $1,219,924; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement with Youth and Family Enrichment Services to increase the maximum obligation by $75,000 to a new maximum of $1,294,924; and

WHEREAS, this Board of Supervisors has been presented with the Amendment to the Agreement and has approved it as to both form and content and desires to enter into the amended Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest to the President’s signature thereto.
