AMENDMENT NO. 1 TO AGREEMENT

BETWEEN THE COUNTY OF SAN MATEO AND
JEFFERSON UNION HIGH SCHOOOL DISTRICT

 

THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and JEFFERSON UNION HIGH SCHOOL DISTRICT, hereinafter called "Contractor";

 

W I T N E S S E T H:

 

WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;

 

WHEREAS, the parties entered into an Agreement for providing a 0.5 Full Time Employee (FTE) Medical Services Assistant II (MSA II) at the Daly City Youth Health Center on May 26, 2009, for the term October 1, 2008, to September 30, 2010, in an amount not to exceed $50,000, said position thereafter being increased to a full-time (1.0 FTE) position; and

 

WHEREAS, the parties wish to amend the Agreement to extend the term by nine months to June 30, 2011, and increase the amount of the Agreement by $80,000, for a new maximum obligation of $130,000.

 

NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:

Section 10, Term of the Agreement, is amended to read as follows:

Subject to compliance with the terms and conditions of this Agreement, the term of this Agreement shall be from October 1, 2008 to June 30, 2011. This Agreement may be terminated by JUHSD or County at any time upon thirty (30) days' written notice to the other party.

Section 2, Payments, Subsection A, Maximum Amount, of the agreement is amended to read as follows:

Maximum Amount. In full consideration of County's performance of the services described in Schedule A, the amount that JUHSD shall be obligated to pay for services rendered under this Agreement shall not exceed ONE-HUNDRED THIRTY THOUSAND DOLLARS ($130,000) for the contract term.

Schedules A and B of the original Agreement are replaced with the revised attached Schedules A and B.

   

4.

All other terms and conditions of the agreement dated May 26, 2009, between the County and Contractor shall remain in full force and effect.
