BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AGREEMENT NO. HI-1011-08 WITH THE CALIFORNIA DEPARTMENT OF AGING FOR THE HEALTH INSURANCE COUNSELING AND ADVOCACY PROGRAM FOR THE TERM JULY 1, 2010 THROUGH JUNE 30, 2011, IN THE AMOUNT OF $272,958; AND 2) AUTHORIZING THE CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE FUNDING AMOUNT BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs pursuant to senior services; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Aging and Adult Services Division shall provide Health Insurance Counseling and Advocacy Program (HICAP) services; and

 

WHEREAS, both parties now wish to enter into an Agreement to provide HICAP funding for community education, lay counseling, and advocacy related to Medicare for the term of July 1, 2010 through June 30, 2011, for a total obligation of $272,958; and

 

WHEREAS, this Board has examined the Agreement and has approved it as to both form and content.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is hereby authorized to execute contract amendments which modify the funding amount by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
