BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH JEFFERSON UNION HIGH SCHOOL DISTRICT TO PROVIDE A MEDICAL SERVICES ASSISTANT II AT THE DALY CITY YOUTH HEALTH CENTER AND EXTENDING THE TERM OF THE AGREEMENT BY NINE MONTHS TO JUNE 30, 2011, FOR A NEW TERM OF OCTOBER 1, 2008 THROUGH JUNE 30, 2011 AND INCREASING THE AMOUNT OF THE AGREEMENT BY $80,000, FROM $50,000 TO A NEW MAXIMUM AMOUNT PAYABLE OF $130,000.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, an Agreement with Jefferson Union High School District (JUHSD) was approved whereby San Mateo Medical Center (SMMC) hired and provided a half-time (0.5 FTE) Medical Services Assistant II (MSA II) at Daly City Youth Health Center (DCYHC) for $50,000, said position thereafter being increased to a full-time (1.0 FTE) position; and

 

WHEREAS, JUHSD has increased the funding and is allocating an additional $80,000 to provide a full-time MSAII to meet the increased needs of the DCYHC providers for an extended term; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Amendment No. 1 to the Agreement, reference to which is hereby made for further particulars, extending the term of the Agreement by nine months to June 30, 2011, and increasing the maximum amount of the Agreement by $80,000 to a new maximum amount payable of $130,000; and

 

WHEREAS, this Board has considered the Amendment to the Agreement and approved it as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized to execute Amendment No. 1 to the Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
