SUBJECT:

Agreement No. MS-1011-13 with the California Department of Aging for Fiscal Year 2010-11

 

RECOMMENDATION:

Adopt a Resolution authorizing the:

   

A)

President of the Board to execute Agreement No. MS-1011-13 with the California Department of Aging, in the amount of $771,300 for the continuation of Multipurpose Senior Services Program services for the term of July 1, 2010 to June 30, 2011; and

   

B)

Chief of the Health System or designee to execute contract amendments which modify the funding amount by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.

   

BACKGROUND:

It is anticipated that a total of 250 clients will be served through the Multipurpose Senior Services Program (MSSP) funding annually. MSSP, one of Aging and Adult Services’ (AAS) case management programs, provides comprehensive social and health care management services to Medi-Cal eligible fragile elders, who are at risk of needing skilled nursing level of care.

 

The primary objectives of MSSP are to avoid the inappropriate placement of fragile older persons in nursing facilities and to foster independent living in their own communities. MSSP provides a care management team of social workers and a public health nurse to assess client eligibility and care need and to arrange and monitor the delivery of services. A client becomes ineligible for MSSP when the program’s services are no longer sufficient to enable the individual to remain safely at home. Though many clients stop participation because of necessary institutionalization, many others are able to avoid institutionalization altogether through MSSP’s services and are able to remain at home as clients with the program for the remainder of their lives.

 

DISCUSSION:

This Agreement includes funds for the MSSP social workers and nurses to provide care management services covering a full range of social and health services. These services include eligibility, assessments, care plan development, service coordination, and service delivery. Staff also provides information, education, counseling, and advocacy to clients and family members. Funds from this Agreement also provide for home modifications, attendant care, respite care, housing assistance, meal services, protective services, and transportation services. Funds can also be used to purchase durable medical equipment and other supplies.

 

County Counsel has reviewed and approved the Agreement and Resolution as to form.

 

This Agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by allowing AAS to provide services to clients in order to avoid inappropriate placements in nursing facilities and to foster independent living. It is anticipated that 22 of the 250 clients served through MSSP in FY 2010-11 will require placement in a skilled nursing facility, which demonstrates how the services provided through MSSP assist in avoiding placement in a skilled nursing facility.

*We are anticipating a slight increase to the number of severely frail clients served by MSSP in FY 2010-11, therefore, there is a potential that more clients may need skilled nursing level care.

 

FISCAL IMPACT:

The term of the Agreement is July 1, 2010 through June 30, 2011. Execution of this Agreement with California Department of Aging allows the County to claim its allocation of MSSP funding for FY 2010-11 in the amount of $771,300. These funds have been included in the tentatively Adopted AAS FY 2010-11 Budget. There is no Net County Cost associated with this Agreement.
