AMENDMENT TO AGREEMENT

BETWEEN THE COUNTY OF SAN MATEO AND
YMCA SAN FRANCISCO YOUTH SERVICES BUREAU FOR PREVENTION AND EARLY INTERVENTION SERVICES

THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and YMCA SAN FRANCISCO YOUTH SERVICES BUREAU, hereinafter called "Contractor";

W I T N E S S E T H:

WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;

WHEREAS, the parties entered into an Agreement for prevention and early intervention services for at-risk youth; and

WHEREAS, the parties wish to amend the Agreement to add $158,897 for a new total obligation of $562,036 and to extend the term of the contract to June 30, 2011;

NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:

Original Exhibit A1 (Scope of Work) is replaced with Revised Exhibit A1 (rev. 5/1/10)

2.

Original Exhibit B (Payment Schedule) is replaced with Revised Exhibit B (rev. 5/1/10)

3.

Original Exhibit C (Program Monitoring) is replaced with Revised Exhibit C (rev. 5/1/10).

4.

All other terms and conditions of the agreement dated August 25, 2009 between the County and Contractor shall remain in full force and effect.
