PYRAMID ALTERNATIVES
SCOPE OF WORK
July 1, 2008 through June 30, 2011
In consideration of the payments as set forth in Exhibit “B,” Contractor shall provide the following services:
A. In accordance with provisions of the California Youth Services Act, Juvenile Probation and Camps Funding, pertaining to the qualifying services, Pyramid Alternatives shall provide a new and expanded comprehensive school-based programs called Strengthening Our Youth. The program shall be consistent with best practices by placing a high value on consultation, collaboration, and joining with youth, families, teachers, probation, and agency partners to identify, assess, and plan the appropriate level of intervention to develop assets in at-risk youth. This approach to school-based and outpatient intervention shall include: 1) prevention and early intervention in schools; 2) treatment in an outpatient setting; and 3) parent engagement.
B. Contractor shall provide services using the philosophy of Youth Asset Development and other evidence-based practices. Based on the Contractor’s assessment of the referred youth, Contractor will strive toward short- and long-term outcomes demonstrating program effectiveness with clients served.

D. Contractor shall provide services to 1,064 unduplicated clients between July 1, 2008 and June 30, 2010. Contractor shall provide services to 500 unduplicated clients between July 1, 2010 and June 30, 2011.
E. Services shall be provided at South San Francisco High School in South San Francisco, Fernando Rivera and Ben Franklin Middle Schools in Daly City, and Half Moon Bay High School in Half Moon Bay.

PAYMENTS AND RATES

In full consideration of services rendered in accordance with Exhibit “A1” herein and Attachment “I” of the original contract and the terms of his Agreement, County shall pay Contractor in the following manner unless otherwise specifically authorized by the Chief Probation Officer or his designee.
1. Contractor shall submit invoices for actual program expenditures according to the following schedule:

These invoices shall be sent to Anessa Farber, Management Analyst, Juvenile Services, 222 Paul Scannell Drive, San Mateo, CA 94402.
2. The maximum amount County shall pay the Contractor shall not exceed FOUR HUNDRED SIXTY ONE THOUSAND SEVEN HUNDRED FORTY EIGHT DOLLARS ($461,748) for the term of the agreement. Yearly total payments shall not exceed:
FY 2008-09 – $165,602
FY 2009-10 – $140,762
FY 2010-11 – $155,384
3. Payment for services provided is contingent upon the availability of County, State, or Federal funds. In the event the State or the Federal government does not appropriate the necessary funds as part of either or both of their budgets, the County shall not be liable for any payment whatsoever, including but not limited to, payments that are based on County funds. The County may terminate this Agreement for unavailability of Federal, State, or County funds.
4. County may withhold all or part of Contractor’s total payment if the Chief Probation Officer or his designee reasonably determines that Contractor has not satisfactorily performed the services as described in Exhibit “A1” of this Agreement and Attachment “I” of the original Agreement.
5. County shall give thirty (30) days prior written notice to Contractor of County’s intent to withhold payment.
6. If County reasonably determines that circumstances warrant immediate action, County may withhold payment immediately, without the thirty (30) day waiting period, upon County’s written notice with justification to Contractor.

PROGRAM MONITORING
July 1, 2008 through June 30, 2011
1. Contractor shall participate in an Evaluation Subcommittee facilitated by Applied Survey Research and San Mateo County Probation to address local and state programming requirements. Relevant intended outcome, objectives, and other evaluation measures shall be discussed and measured. Contractor shall participate fully in all evaluation techniques agreed to by the Subcommittee, including but not limited to a logic model, evaluation plan, and a variety of collection methods. Contractor shall submit individual-level demographic, service level/dosage, and outcome data semi-annually or as needed by Applied Survey Research for local and state requirements.
2. Contractor shall submit the following to County:
a. Supporting salary and benefits documentation for each JPCF-funded staff and receipts for all operational costs;
b. Monthly Provider Time Study Forms for each JPCF-funded staff;
c. Quarterly Units of Service report showing the actual delivery of services, including current and year-to-date of:
• Number of unduplicated youth served
• Number of family members served
• Number of units of service provided by service type as described in Attachment III of the original Agreement
Direct service (Units = number of participants at a session x number of hours per session x number of staff involved in activity)
Case management (1 unit = 1 hour of case management)
d. Quarterly summary of program service highlights, identified problems, solutions, and goals.

3. County Responsibilities
a. Coordinate with Applied Survey Research to review and monitor all correspondence and reports submitted by Contractor;
b. Meet with Program Director and staff as requested;
c. View sites as needed during the term of this Agreement; and
d. Assure Probation participation in evaluation subcommittee work.
