SUBJECT:

Agreement with the City and County of San Francisco

 

RECOMMENDATION:

Adopt a Resolution authorizing the:

A)

President of the Board to execute an Agreement with the City and County of San Francisco for funding to provide primary health care and psycho-social services to individuals with HIV/AIDS for the period of March 1, 2010 through February 28, 2011, for a maximum obligation of $2,030,578; and

   

B)

Chief of the Health System or designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.

 

BACKGROUND:

The purpose of the Ryan White Comprehensive AIDS Resources Emergency Act of 1990 (CARE) is to provide emergency assistance to eligible metropolitan areas that are disproportionately affected by HIV/AIDS and to make financial assistance available for the delivery of essential services to individuals and families with HIV/AIDS. CARE funds come to the San Mateo County STD/HIV Program (SMCSH) through an agreement with the City and County of San Francisco (CCSF). The amount of funding SMCSH receives each year is determined by the number of persons with HIV/AIDS living in San Mateo County.

 

DISCUSSION:

CARE funding is utilized to pay for a continuum of services provided to residents of San Mateo County affected by HIV/AIDS, which include primary medical care, medical case management, dental care, mental health, substance abuse, housing and food services.

Primary medical care, medical case management and mental health services are provided at the Edison Clinic and most of the supportive services are provided by other County departments and community-based organizations. Due to the uncertainty and lateness of the authorization of federal funding, CCSF did not send the Agreement to the County until the beginning of August 2010. The work required by the terms of the Agreement has been undertaken by the Health System on written assurance and direction of CCSF.

 

The Agreement and Resolution have been reviewed and approved by County Counsel.

 

Approval of this Agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing medical care and psycho-social services to individuals affected by HIV/AIDS. It is anticipated that the viral load of 75% of clients receiving services will decrease or remain undetectable as a result of the medical and psycho-social services received.

FISCAL IMPACT:

The term of the Agreement is March 1, 2010 through February 28, 2011. The maximum amount of funding for the Agreement is $2,030,578, of which $676,859 was included in the Community Health FY 2009-10 Adopted Budget, and $1,353,719 is included in the Community Health tentatively Adopted FY 2010-11 Budget. CARE requires a Maintenance of Effort of $439,996, which has been met by the allocated Net County Cost for the STD/HIV Program.
