RESOLUTION AUTHORIZING THE: A) PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH FOR THE IMMUNIZATION ASSISTANCE PROGRAM FOR THE TERM OF JULY 1, 2010 THROUGH JUNE 30, 2011, IN THE AMOUNT OF $283,911; AND B)CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE FUNDING AMOUNT BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance of an Agreement, reference to which is hereby made for further particulars, whereby the California Department of Public Health (CDPH) shall provide funding to San Mateo County for immunization assistance, perinatal hepatitis B prevention, and immunization registry activities in the amount of $283,911 for the term of July 1, 2010 through June 30, 2011; and

WHEREAS, this Board has examined and approved it as to both form and content and desires to enter into said Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term of services is/are within the current or revised fiscal provisions.
