RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A FOURTH AMENDMENT TO THE AGREEMENT WITH OUR COMMON GROUND, INC., FOR THE PROVISION OF ALCOHOL AND OTHER DRUG SERVICES, INCREASING THE MAXIMUM OBLIGATION TO $5,574,401 WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2009 THROUGH JUNE 30 |
RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that |
WHEREAS, on June 23, 2009, this Board of Supervisors approved an Agreement with Our Common Ground, Inc. for the provision of alcohol and drug services for the term of July 1, 2009 through June 30, 2011, for a maximum obligation of $5,971,818; and |
WHEREAS, January 15, 2010, this Board of Supervisors approved a First Amendment to the Agreement to add funding for Second Chance Act Re-entry Residential Services in the amount of $37,542, and increase County funding for Adolescent Residential Room and Board services by $100,000, for a new maximum obligation of $5,380,657, with no change to the Agreement term; and |
WHEREAS, April 13, 2010, this Board of Supervisors approved a Second Amendment to the Agreement to decrease the Drug Medi-Cal (DMC) rate, and add funding in the amount of $5,891 for a new maximum obligation of $5,243,115, with no change to the Agreement term; and |
WHEREAS, July 15, 2010, this Board of Supervisors approved a Third Amendment to the Agreement to incorporate description of services for the Mental Health Services Act (MHSA), reconcile services and funding provided during FY 2008- 09, and reduce the NRC Adult Residential by $17,456, for a new maximum obligation of $5,363,201 with no change to the Agreement term; and |
WHEREAS, there has been presented to this Board of Supervisors a Fourth Amendment to the Agreement whereby to increase Second Chance Act Re-Entry Services to include outpatient and intensive day treatment, increasing the maximum obligation by $24,200, for a new maximum obligation of $5,387,401; to increase County funding for Adolescent Residential Room and Board services by $150,000, for a new maximum obligation of $5,537,401; to increase Second chance Act Re-Entry Services for Residential Services increasing the maximum obligation by $37,000, for a new maximum obligation of $5,574,401 dollars; and Amend the Facility Use Agreement, and no change to the Agreement term; and |
WHEREAS, this Board has been presented with a form of the Fourth Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into this Amended Agreement. |
NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Fourth Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto. |
