BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE: A) PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH YOUTH LEADERSHIP INSTITUTE, TO PROVIDE TOBACCO EDUCATION RELATED TO SMOKING IN FILMS AND A PUBLIC EDUCATION CAMPAIGN FOR THE TERM OF JULY 1, 2009 THROUGH JUNE 30, 2011, AND INCREASE THE MAXIMUM OBLIGATION BY $70,000 FROM $179,558 TO A NEW MAXIMUM OBLIGATION OF $249,558; AND B) CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 9, 2009, the Board approved an Agreement with Youth Leadership Institute for the term of July 1, 2009 through June 30, 2011 in the amount of $179,558 for alcohol and drug prevention services for; and

 

WHEREAS, both parties wish to amend the Agreement for the Contractor to provide tobacco education related to smoking in films and a public education campaign on the impacts of secondhand smoke in housing, for the term of July 1, 2009 through June 30, 2011, and increase the Agreement maximum obligation by $70,000 from $179,558 for a new maximum obligation of $249,558.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 
 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
