RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO ACCEPT AN AWARD FROM THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH, TUBERCULOSIS CONTROL BRANCH FOR TUBERCULOSIS PREVENTION AND CONTROL ACTIVITIES FOR THE TERM OF JULY 1, 2010 THROUGH JUNE 30, 2011, IN THE AMOUNT OF UP TO $309,197 INCLUDING AN ALLOTMENT OF UP TO $26,500 FOR FOOD, SHELTER, INCENTIVES AND ENABLERS EXPENDITURES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Acceptance of Award from the California Department of Public Health, reference to which is hereby made for further particulars; and

WHEREAS, the California Department of Public Health will provide funding to support the Tuberculosis Program for the period of July 1, 2010 through June 30, 2011, for a total amount of up to $309,197; and

WHEREAS, this Board has been presented with a form of the Acceptance of Award and has examined and approved it as to both form and content and desires to execute the Acceptance of Award.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Acceptance of Award for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
