RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH PYRAMID ALTERNATIVES, INC., FOR THE PROVISION OF ALCOHOL AND OTHER DRUG SERVICES, INCREASING THE AMOUNT BY $24,200 FOR A MAXIMUM OBLIGATION OF $4,382,451, WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2009 THROUGH JUNE 30, 2011

 
 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 16, 2009, this Board approved an Agreement with Pyramid Alternatives, Inc., for the provision of alcohol and drug services, and authorizing the Chief of the Health System or designee to execute contracts amendments which modify the funding amount by no more than $25,000 in aggregate, for the term of July 1, 2009 through June 30, 2011, for a maximum obligation of $4,383,046; and

 

WHEREAS, on August 18, 2010, the Chief of the Health System approved a First Amendment to the Agreement to decrease funding by $24,795 for a new maximum obligation of $4,358,251; with no change to the Agreement term; and

 

WHEREAS, there has been presented to this Board a Second Amendment to the Agreement whereby to reduce the administrative fee to the First Offender Program from 10% to 8%, and to increase Second Chance Act Re-Entry Services by $24,200, to include outpatient and intensive day treatment services, increasing the maximum obligation to $4,382,451, with no change to the Agreement term; and

 

WHEREAS, this Board has been presented with the Second Amendment to the Agreement and has approved it as to both form and content and desires to enter into this amended Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
