BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A FOURTH AMENDMENT TO THE AGREEMENT WITH PROJECT NINETY FOR THE PROVISION OF ALCOHOL AND OTHER DRUG SERVICES, INCREASING THE MAXIMUM OBLIGATION BY $24,200 TO $6,525,593, WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2009 THROUGH JUNE 30, 2011

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 23, 2009, this Board approved an Agreement with Project Ninety, Inc., for the provision of alcohol and drug services, and authorizing the Chief of the Health System or designee to execute contract amendments which modify the funding amount by no more than $25,000 in aggregate for the term of July 1, 2009 through June 30, 2011, for a maximum obligation of $6,380,746; and

 

WHEREAS, on April 13, 2010, this Board approved a First Amendment to the Agreement to incorporate Second Chance Act Re-Entry Residential Services and increase the maximum obligation by $182,480 for a new maximum obligation of $6,563,226, with no change to the Agreement term; and

 

WHEREAS, on May 18, 2010, the Chief of the Health System approved a Second Amendment to the Agreement to increase Bay Area Service Network services by $6,567 to a new maximum obligation of $6,569,793, with no change to the Agreement term; and

 

WHEREAS, on July 15, 2010, the Chief of the Health System approved a Third Amendment to the Agreement to decrease the maximum obligation by $68,400 for a new maximum obligation of $6,501,393, with no change to the Agreement term; and

 

WHEREAS, there has been presented to this Board a Fourth Amendment to the Agreement whereby Second Chance Act Re-Entry Services are being increased by $24,200, to include outpatient and intensive day treatment, increasing the maximum obligation to $6,525,593, with no change to the Agreement term; and

 

WHEREAS, this Board has been presented with a form of the Fourth Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into this amended Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
