BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A THIRD AMENDMENT TO THE AGREEMENT WITH THE LATINO COMMISSION FOR THE PROVISION OF ALCOHOL AND OTHER DRUG SERVICES, DECREASING THE MAXIMUM OBLIGATION BY $595 TO $4,993,023, WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2009 THROUGH JUNE 30, 2011

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 16, 2009, this Board approved an Agreement with The Latino Commission for the provision of alcohol and other drug services, and authorizing the Chief of the Health System or designee to execute contracts amendments which modify the funding amount by no more than $25,000 in aggregate, for the term of July 1, 2009 through June 30, 2011, for a maximum obligation of $4,897,934; and

 

WHEREAS, on April 13, 2010, this Board approved a First Amendment to the Agreement to incorporate Second Chance Act Re-entry Residential Services increasing the maximum by $81,600, to a new maximum obligation of $4,979,534, with no change to the Agreement term; and

 

WHEREAS, on May 18, 2010, the Chief of the Health System approved a Second Amendment to the Agreement to increase funding for the Bay Area Services Network and day treatment services under Comprehensive Drug Court Implementation by $14,084 to a maximum obligation of $4,993,618, with no change to the Agreement term; and

 

WHERAS, there has been presented to this Board a Third Amendment to the Agreement whereby to decrease funds paid during FY 2008-09 by $24,795, and increase funding for Second Chance Act Re-Entry Services by $24,200 for a new maximum obligation of $4,993,023, with no change to the Agreement term; and

 

WHEREAS, this Board has been presented with a form of the Third Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the amended Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Third Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
