RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH SITIKE COUNSELING CENTER FOR THE PROVISION OF ALCOHOL AND OTHER DRUG SERVICES, INCREASING THE MAXIMUM OBLIGATION BY $24,200 TO $4,241,501, WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2009 THROUGH JUNE 30, 2011

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 16, 2009, the Board approved an Agreement with Sitike Counseling Center, for the provision of alcohol and drug services, and authorized the Chief of the Health System or designee to execute contract amendments which modify the funding amount by no more than $25,000 in aggregate, for the term of July 1, 2009 through June 30, 2011, for a maximum obligation of $4,228,840; and

WHEREAS, on July 15, 2010, the Chief of the Health System approved a First Amendment to the Agreement to decrease funding by $11,539 to a new maximum obligation of $4,217,301; with no change to the Agreement term; and

WHEREAS, there has been presented to this Board a Second Amendment to the Agreement whereby increasing Second Chance Act Re-Entry Services by $24,200 to include outpatient and intensive day treatment, increasing the maximum obligation to $4,241,501, with no change to the Agreement term; and

WHEREAS, this Board has been presented with a form of the Second Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into this amended Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
