RESOLUTION AUTHORIZING THE: A) PRESIDENT OF THE BOARD TO EXECUTE TWO AGREEMENTS WITH RAVENSWOOD FAMILY HEALTH CENTER: (1) PROVIDING DENTAL HEALTH CARE FOR HOMELESS INDIVIDUALS, FOR A MAXIMUM OBLIGATION OF $150,000; AND (2) PROVIDING PRIMARY MEDICAL CARE FOR HOMELESS INDIVIDUALS FOR A MAXIMUM OBLIGATION OF $271,371, EACH FOR A TERM OF NOVEMBER 1, 2010 THROUGH OCTOBER 31, 2013; AND B) CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, San Mateo Medical Center (SMMC) was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Health Care of the Health and Resources Services Administration to provide primary, dental, and behavioral health care to San Mateo County’s homeless population; and

 

WHEREAS, In July 2010 SMMC conducted a Request for Proposals (RFP) process for the Homeless Program – Primary Care (fixed site and mobile) and Dental Care (fixed site) and four proposals, two in each category, were received and evaluated; and

 

WHEREAS, Each responder provided unique capacity in service delivery or in reaching target populations and Ravenswood Family Health Center was selected; and

 

WHEREAS, both the County and Ravenswood Family Health Center wish to enter into the Agreements, reference to which is hereby made for further particulars, whereby Ravenswood Family Health Center shall provide dental health care for homeless individuals in the amount of $150,000 and provide primary medical care for homeless individuals in the amount of $271,371 each for a term of November 1, 2010 through October 31, 2013; and

 

WHEREAS, this Board has examined the Agreements and has approved them as to both form and content.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
