RESOLUTION AUTHORIZING THE: A) PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT ONE TO THE AGREEMENT WITH PETDATA, INC. TO PROVIDE ANIMAL LICENSING SERVICES, EXTENDING THE TERM THROUGH JUNE 30, 2013, AND INCREASING THE AGREEMENT MAXIMUM BY $333,000 TO A NEW MAXIMUM OF $813,000; AND B) CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on December 18, 2007, the Board approved an Agreement with PetData, Inc. for the term of January 1, 2008 through December 31, 2010, in the amount of $480,000 to provide animal licensing services; and

WHEREAS, both parties wish to amend the Agreement with PetData Inc. for animal licensing services, to increase the Agreement maximum obligation by $333,000 from $480,000 for a new maximum obligation of $813,000, and extend the term through June 30, 2013.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
