RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A FIRST AMENDMENT TO THE AGREEMENT WITH YOUTH AND FAMILY ENRICHMENT SERVICES FOR THE PROVISION OF EARLY CHILDHOOD COMMUNITY TEAM SERVICES; INCREASING THE MAXIMUM OBLIGATION BY $108,518 TO A NEW MAXIMUM OF $1,456,169, WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2010 THROUGH JUNE 30, 2011.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 14, 2010, this Board approved an Agreement with Youth and Family Enrichment Services, for the provision of outpatient mental health services, support for the youth development initiative, and community worker services and authorizing the Chief of the Health System or designee to execute contracts amendments which modify the funding amount by no more than $25,000 in aggregate, for the term of July 1, 2010 through June 30, 2011, for a maximum obligation of $1,347,651; and

WHEREAS, there has been presented to this Board of Supervisors a First Amendment to the Agreement to incorporate Early Childhood Community Team Services, increasing the maximum obligation by $108,518, for a new maximum of $1,456,169, with no change to the Agreement term; and

WHEREAS, this Board has been presented with a form of the First Amendment

and has examined and approved it as to both form and content and desires to enter into this Amended Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the

President of this Board of Supervisors be and is hereby authorized and directed to

execute said First Amendment for and on behalf of the County of San Mateo, and the

Clerk of this Board shall attest the President’s signature thereto.
