Agreement with Shelter Network of San Mateo County

 

RECOMMENDATION:

Adopt a Resolution authorizing the:

A)

President of the Board to execute an Agreement with Shelter Network of San Mateo County to provide case management services to San Mateo County’s homeless population for the term of November 1, 2010 through October 31, 2013, for a maximum obligation of $276,750; and

 

B)

Chief of the Health System or designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.

 

BACKGROUND:

San Mateo Medical Center (SMMC) was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Health Care of the Health and Resources Services Administration to provide primary medical, dental, and behavioral health care to San Mateo County’s homeless population. The HCH Program is designed to assist homeless adults, families and youth with access to and utilization of SMMC’s continuum of care and other support services. The HCH Program contracts with both County and community-based programs to provide this care.

 

In August 2010 SMMC conducted a Request for Proposals (RFP) process for the Homeless Program – Enabling Services. Four proposals were received and evaluated. The proposal from Shelter Network of San Mateo County (Shelter Network) was one of the two selected for funding.

 

DISCUSSION:

Shelter Network was selected to receive HCH funding to provide case management, outreach, patient and community education, transportation, eligibility assistance, translation services, and referral services to improve client access to San Mateo County Health System primary medical services. Shelter Network will provide comprehensive enabling services to at least 450 unique homeless individuals each year, 150 of whom must not have been in the SMMC/HCH system the previous year. In addition, a minimum of 100 of these homeless individuals will complete a Supplemental Security Income (SSI) Medi-Cal or Social Security Disability Insurance (SSDI) Medicare application each contract year in an effort to remove additional barriers from accessing services. Due to the complexity of the negotiations involved there was a delay in processing this Agreement.

 

The Agreement and Resolution have been reviewed and approved by County Counsel as to form. The Contractor’s insurance has been reviewed and approved by Risk Management. The Contractor has assured compliance with the County’s Contractor Employee Jury Service Ordinance, as well as all other contract provisions that are required by County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.

 

This Agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing access to case management services to homeless individuals residing at shelters and to unsheltered homeless individuals residing on the streets of San Mateo County. It is anticipated that 75% of homeless individuals screened will access primary care (medical, dental, behavioral) at least once during the contract year through the San Mateo County Health System. By accessing primary care and receiving ongoing case management services these individuals will experience an improvement in their overall health.

FISCAL IMPACT:

The term of the Agreement is November 1, 2010 through October 31, 2013. The maximum amount payable under the Agreement is $276,750. All funds for this Agreement are provided by the HCH grant. Expenditures and grant revenue of $60,000 for this Agreement is included in the SMMC FY 2010-11 Adopted Budget. Grant revenue will be included in the SMMC FY 2011-12 Recommended Budget, and future budgets, for the remainder of the Agreement. There is no Net County Cost.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC.
