BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE: A) PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH SHELTER NETWORK OF SAN MATEO COUNTY TO PROVIDE CASE MANAGEMENT SERVICES TO SAN MATEO COUNTY’S HOMELESS POPULATION FOR THE TERM OF NOVEMBER 1, 2010 THROUGH OCTOBER 31, 2013, FOR A MAXIMUM OBLIGATION OF $276,750; AND B) CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, San Mateo Medical Center (SMMC) was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Care of the Health and Resources Services Administration to provide primary, dental, and behavioral health care to San Mateo County’s homeless population; and

WHEREAS, Shelter Network of San Mateo County (Shelter Network) has been selected by the County as a community-based program to receive HCH funding to provide homeless adults, families and youth with case management, outreach, patient and community education, transportation, eligibility assistance, translation services, and referral services to improve access to San Mateo County Health System primary medical services; and

WHEREAS, the County and Shelter Network wish to enter into an Agreement under which Shelter Network will provide such case management services to San Mateo County’s homeless population for the term of November 1, 2010 through October 31, 2013, in an amount not to exceed $276,750; and

WHEREAS, this Board has been presented with a form of such Agreement, has examined and approved it as to both form and content, and desires to enter into it.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee is authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
