RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A THIRD AMENDMENT TO THE AGREEMENT WITH CAMINAR FOR THE PROVISION OF ADDITIONAL TECHNICAL ASSISTANCE AND FOR EXPENSES INCURRED BY THE LINDEN STREET FIRE INCREASING THE MAXIMUM OBLIGATION BY $117,937 TO A NEW MAXIMUM OF $6,819,665, WITH NO CHANGE TO THE TERM OF JULY 1, 2010 THROUGH JUNE 30, 2011

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 8, 2010, this Board approved an Agreement with Caminar, by Resolution 070815, for the provision of housing and housing-related services, supported education services, intensive case management services, and rehabilitation services, for a maximum obligation of $6,568,875; and

 

WHEREAS, on September 2, 2010, the Chief of the Health System approved an Amendment to the Agreement increasing the Agreement maximum by $25,000 to provide consultation services, for the implementation of the new Behavioral Health and Recovery Services Electronic Medical Record software Avatar, to a new maximum of $6,593,875 with no change to the Agreement term; and

 

WHEREAS, on January 11, 2011, this Board approved a Second Amendment, by Resolution 071211, for the provision of additional technical assistance, and adding the “Seeking Safety” service model, increasing the maximum obligation by $107,853 to a new maximum of $6,701,728 with no change to the Agreement term; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Third Amendment, reference to which is hereby made for further particulars, to increase the maximum obligation by $117,937 to a new maximum of $6,819,665 with no change to the Agreement term; and

 

WHEREAS, the Board of Supervisors has examined and approved the Third Amendment as to form and content and desires to enter into the Third Amendment to the Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
