AMENDMENT TO AGREEMENT

BETWEEN THE COUNTY OF SAN MATEO AND
PAUL LINQUIST, MD

 

THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and Paul Linquist, MD, hereinafter called "Contractor";

 

W I T N E S S E T H:

 

WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;

 

WHEREAS, the parties entered into an Agreement with Paul Linquist, MD for orthopedic services on May 5, 2009; and

 

WHEREAS, the parties wish to amend the Agreement to increase the maximum amount payable thereunder by $35,000, from $250,000 to $285,000.

 

NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:

Section II of Exhibit A, Services, of the Agreement is amended in its entirety to read as follows:

For the period from April 1, 2009 through March 31, 2011, Contractor will provide:

A. One half-day clinic, four (4) hours each, with a target of 20 patients seen in

each half-day clinic, every other week.

B. One half-day surgery block, approximately four (4) hours each, every other

week.

C. One day of call every other week, including inpatient, ambulatory, and

Emergency Department consults and all follow-up from call, including

operative cases.

D. A monthly call coverage schedule for the orthopedic department.

E. The position of Chief of Orthopedics.

For the period January 1, 2011 through March 31, 2011, Contractor may provide:

F. One half-day clinic blocks per week, four (4) hours each, in addition to the

clinic services set forth in Section II.A of this Exhibit A.

G. One half-day surgery block per week, approximately four (4) hours each, in

addition to the surgery services set forth in Section II B. of this Exhibit A.

Section III of Exhibit B, Payments, of the Agreement is amended to read in its entirety as follows:

III. Contractor compensation for orthopedics by County is based on a projected volume of 2290 RVU work units per annum. If the contract terms of service are amended such that the current number of clinic sessions and surgery blocks in Exhibit A are changed, the projected volume of RVU work units and corresponding compensation shall be adjusted accordingly.

If the average monthly work units for Contractors productivity in orthopedics (based on 2290 annual units) vary by fifteen percent (15%) over a six (6) month period, the Chief Medical Officer or designee will review and discuss the compensation metric with Contractor, and it will be changed if appropriate.

Additional clinic and/or surgery blocks as described in Exhibit A, Section II, will be paid monthly at a rate of $1,800 per block.

VI. The term of this Agreement is April 1, 2009 through March 31, 2011. Total payment for services performed under this Agreement shall not exceed TWO HUNDRED EIGHTY FIVE THOUSAND DOLLARS ($285,000).

All other terms and conditions of the Agreement dated May 5, 2009, between the County and Contractor shall remain in full force and effect.
