SUBJECT:

Amendment One to the Agreement with Self Help for the Elderly

 

RECOMMENDATION:

Adopt a Resolution authorizing the President of the Board to execute Amendment One to the Agreement with Self Help for the Elderly increasing the total amount by $43,198 to $464,835 with no change to the term of July 1, 2010 through June 30, 2011.

 

BACKGROUND:

On September 28, 2010, your Board authorized Resolution 71056 approving an agreement with Self Help for the Elderly and 22 other providers for Adult Day Care/Adult Day Health Care, Alzheimer’s Day Care Services, Case Management, Congregate Nutrition, Family Caregiver Support, Health Insurance Counseling and Advocacy Program (HICAP), Health Promotion/Disease Prevention, Home-Delivered Meals, Legal Assistance, Medication Management, Ombudsman Services, Senior Employment and Transportation. The total amount of the 23 Agreements was $3,066,516 for the term of July 1, 2010 through June 30, 2011. This Resolution also authorized the Chief of the Health System or designee to execute limited contract amendments.

 

On December 21, 2010, the Chief of the Health System approved Amendment One to the State Agreement MI-1011-08-1 which increased federal funding for the Medicare Improvements for Patients and Providers Act (MIPPA) in the amount of $447 for a total increase to $24,433. There was no change to the term August 1, 2009 through May 31, 2011.

 

On January 25, 2011, an amendment to agreement HI-1011-08 with the California Department of Aging (CDA) and an Appropriation Transfer Request (ATR) was brought before your Board authorizing the acceptance of additional one-time-only federal funding in the amount of $28,183 for a total increase to $301,141.

 

On February 15, 2011, your Board authorized agreement 2M-1011-08 with CDA in the amount of $40,604 for additional MIPPA services (2 MIPPA) for the term January 1, 2011 through June 30, 2012. An ATR in the amount of $20,302 was included for the acceptance of the funding for the term January 1, 2011 through June 30, 2011.

 

DISCUSSION:

 

Aging and Adult Services (AAS) contracts with Self Help for the Elderly to provide HICAP and MIPPA services. HICAP is a volunteer-supported program that provides three basic services: community education, lay counseling, and advocacy related to Medicare. MIPPA services provide additional Medicare Part D outreach and are targeted to low-income residents. Self Help for the Elderly was selected to provide HICAP services through a Request for Proposals process conducted in December 2008 for the contract term July 1, 2009 through June 30, 2013. The MIPPA services were added to the Agreement due to the new Federal funding available. This increased funding allows Self Help for the Elderly to provide additional HICAP and MIPPA services.

 

HICAP is an integral component of California’s community-based long-term care system. Medicare beneficiaries and those under 65 who are about to become eligible for Medicare or who are disabled and on Medicare are eligible for HICAP services. Those who are low-income and eligible for Medicare Part D are eligible for MIPPA services.

 

County Counsel has reviewed and approved the Resolution and Amendment as to form and content.

 

This Amendment contributes to the Shared Vision 2025 outcome of a Healthy Community by allowing AAS to continue to provide clients with objective health insurance counseling, advocacy and assistance. It is anticipated that 75% of HICAP/MIPPA services recipients will feel that they have gained useful knowledge and feel that their personal life has improved as a result of the services received. In FY 2009-10 Self Help for the Elderly exceeded the expectations of 75% in the following performance measures.

FISCAL IMPACT:

The term of the Agreement remains the same, July 1, 2010 through June 30, 2011. This Amendment increases the total amount by $20,995 in HICAP, $22,203 in MIPPA and 2 MIPPA funds for a total increase of $43,198 from $421,637 to $464,835. There is no Net County Cost associated with this action. Funds for FY 2011-12 will be included in AAS FY 2011-12 Recommended Budget.
