Position Changes at San Mateo Medical Center

RECOMMENDATION:
|
Adopt an Ordinance amending the Master Salary Ordinance to add and delete positions in the San Mateo Medical Center division of the Health System.
|
|
BACKGROUND:
|
A survey conducted by the California Department of Public Health (CDPH) on March 3, 2010, determined that the San Mateo Medical Center (SMMC) was not in compliance with Title 22, because the hospital pharmacy did not have overall responsibility for pharmaceutical services at Burlingame Long-Term Care (BLTC) center and Long Term Care (LTC) unit on the first floor at the main campus. In addition to filling prescription, over-the-counter and intravenous sterile products for the residents of LTC, Title 22 requires that a qualified pharmacist consultant conduct medication regimen review for each resident at least every 30 days.

DISCUSSION:
|
For many years, pharmaceutical services and consultant pharmacist services for the LTC facilities has been outsourced to PharMerica, a pharmacy service that specializes in long-term care. Although the facilities have been surveyed many times, the survey teams have not identified this arrangement as a deficiency until CDPH conducted its Medication Error Reduction Plan (MERP) survey from February 23, 2010 through March 3, 2010.

The Clinical Services Manager of the Pharmacy at SMMC had discussions with the Chief Pharmaceutical Consultant for CDPH and the Pharmacist Consultant who performed the survey regarding this deficiency. In the Plan of Correction that was accepted in December of 2010, SMMC Pharmacy will assume pharmaceutical services for LTC 1A on February 1, 2011, and for BLTC on July 1, 2011.

The Salary Ordinance amendment has been reviewed and approved by the County Counsel.

This Amendment contributes to the Shared Vision 2025 of a Healthy Community by providing compliant pharmaceutical care to the residents of Burlingame Long-Term Care and of The Long-Term Care unit at SMMC. It is anticipated that 198,000 outpatient prescriptions will be filled by the pharmacy.

FISCAL IMPACT:
|
This action represents an estimated increase in annual salary and benefits of $559,350, an estimated increase in annual drug costs of $950,000, and an estimated annual increase in supply costs of $91,600. The results of SMMC Pharmacy filling and billing for residents of BLTC and LTC1A is estimated to be $1,755,000. The results represent an estimated annual net income of $220,000.

The increased cost and accompanying revenue will be included in the SMMC FY 2011-12 Recommended Budget. Expenses at SMMC are covered by fees for services or third-party payors, whenever possible. Expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC.
